This registration form will be utilized by the TPA or Plan Sponsor to receive Sav-Rx pharmacy data required to meet the Consolidated Appropriations Act (CAA) Section 204 transparency data submission requirements. Data submissions through HIOS (CMS portal) are required by December 27, 2022 for the 2020 and 2021 calendar years. Data file D1 will be provided by the Plan. Data file D2 will be provided by your Medical Plan. Sav-Rx will supply the pharmacy specific data elements and information for Data file P2 and the Narrative Response. The remaining requirements/fields in file P2 and the Narrative must be supplied by the Plan to complete these files. Data files D3, D4, D5, D6, D7 and D8 will be provided completely by Sav-Rx for direct submission through HIOS. Sav-Rx advises that all files be reviewed and outstanding data or information be completed prior to submission by the Plan or Plan's designee to ensure a successful filing with CMS. It is the responsibility of the Plan to obtain and submit any additional data needed by previous PBM or Medical partners to support the entirety of the CAA submission.
